How to mark off Critical Info in Patient Medical History
Patient History can be quite comprehensive and is usually taken down during the Initial Visit of a Patient. From thereon follow-ups are usually much shorter. During Followups it is not practical to read through the entire history each time a Patient visits. What is required is a quick way to scan through critical bits of Patient History.
For example, During history taking you might ask the patient whether the patient suffers from any one of the several chronic conditions. The Patient may answer in affirmative to only a few e.g. Yes to Diabetes, but no to HTN, IHD, TB, and Asthma. Next time the patient comes to you, you want Diabetes to appear in the Patient’s Medical Summary before you start your consult.
